If you use a form to define all the parameters your client has to meet their requirements for candidates, would you mind sharing?

As a new and independent Recruiter, it seems appropriate to have the client define their requirements for filling their position.  Therefore it seemed logical to have them complete a form to aid the recruiter finding the right candidates.  Rather than reinvent the wheel, I was hoping to glean for your experience if you would be willing to share your knowledge.
