How do you identify a 'hallway' file when vetting a candidate

Here's a recurring problem we're facing for cleared candidates.  Great candidate, good mix of skills and seems like a good fit...only to find out after all the time developing that recruiter/candidate rapport, that the management team and or government end customer has a 'hall way file' on the person from past experiences and will not consider them for placement.    Looking for techniques to help drive those out with the candidate other than "anything  you wish to confess before I submit you to the leadership team?"  Anyone else running into that problem?
